A valid Gold or White card along with a referral (Form D904) from your treating GP is required. You do not need to prove the condition is service related (non-liability health cover) to access social work services.
Treatment cycles consist of twelve sessions which can either be utilised as needed or over the course of a twelve month period, which ever comes first. Should additional support be requested, a new referral is required.
DVA has strict requirements for accessing allied health services under the above referral pathway. At the commencement of a treatment cycle, a patient care plan is developed that outlines patient goals, management strategies and session frequency. At the conclusion of a treatment cycle, an end of treatment report is written utilising the patient care plan information, whether goals were achieved, treatments provided and recommendations. The end of treatment report is then sent to the referring GP.
DVA may request a treatment outcome report at the conclusion of the treatment cycle. Should this be the case, the requirements of the report will be discussed in advance and developed collaboratively.

For the Redress Scheme:
You will need to have an accepted Redress offer. If you are an existing client, you may contact the Redress team and nominate us as your preferred service. For new clients, the Redress team will provide you a list of available practitioners within your chosen area , alternatively you may contact us directly to check availability.
As part of the Redress Scheme, acceptance of a formal offer entitles the individual to an initial 20hours of counselling support. Additional hours are available via written request. You may also be eligible for up to 20hours per calendar year thereafter.
For Victims Assist:
If you have experienced a violent crime, you may be eligible to access support through Victims Assist. To access counselling support please contact us directly as additional details will need to be provided to Victims Assist.
Counselling is either billed directly to Victims Assist or the individual can pay the invoice and seek reimbursement from Victims Assist.
All counselling services are confidential however a progress report may be required to support ongoing access to counselling. Should this be required, this will be discussed in advance and developed collaboratively.

To access counselling or social work support, a valid Work Cover Qld claim is required.
Unless otherwise stated, counselling under Work Cover Qld requires prior approval. Please contact us directly to discuss accessing counselling and rehabilitation support through Embrace the Journey.

To access a parenting capacity assessment, a valid referral is required. Embrace the Journey accepts referrals from the Courts, Child Safety, Department of Corrective Services and NDIS.
Psychosocial capacity assessments may be requested by Child Safety, NDIS or Qld Health to support case planing, family reunification or goal development.
Therapeutic life story work requires a valid referral from Child Safety or Youth Justice.
